Clinical Pharmacist (Healthcare)



Work Schedule: 

This is a part-time, FTE .80, 1st shift position in the Anticoagulation Clinic. Monday - Friday AM hours. Remote work location. No PM. No weekend requirements. Hours may vary based on department needs.

Join our team of respected experts in medication management. 

We are seeking a Clinical Pharmacist to:

  • Provide medication management services to patients in the UW Health Anticoagulation Clinic to assure that anticoagulation therapy is safe, effective, and cost-effective.
  • Autonomously participate in delegation protocol-driven medication management and program elements to optimize patient care.
  • Actively collaborate with interdisciplinary patient care team and analyze clinical outcomes of applicable patient populations to continuously improve processes and care.
  • Provide training and education to pharmacy residents and PharmD students through practical and didactic experiences. 

Prior experience working with patients taking anticoagulation therapies highly desired. PGY1 Residency or PGY2 in Ambulatory Care Residency preferred. CACP or BCACP preferred but not required.

Education:

Minimum – Bachelor's degree in Pharmacy or a PharmD degree

Preferred – Completion of an ASHP accredited Residency training program

Work Experience:

Minimum – One year of clinical patient care and orders management experience

Preferred – Three years of ambulatory care or integrated health system experience or completion

of an ASHP accredited residency training program

Licenses and Certifications:

Minimum – Licensed to practice pharmacy in the State of Wisconsin or must obtain within 90 days of

hire date.

Preferred – Certification through the appropriate certifying program (Board of Pharmacy Specialties,

National Board of Nutrition Support Certification, National Certification Board for

Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.).

UW Health Pharmacy Services Building – a non-clinical location in Middleton that houses nearly 100 Pharmacy professionals. These employees perform various services and duties, including pharmacy administration, Pharmacy Technician Training Program, pharmacy primary care services, medication prior authorization, sterile and non-sterile compounding, specialty/mail service program and call center.
