Behavioral Health Case Manager PACT (Personal Services)



KMHS is Kitsap's nonprofit, designated community mental health provider for adults with acute and/or chronic serious mental illnesses, and children with serious emotional disturbances and their families. During 2016, KMHS provided services to more than 6,800 children and adults. Behavioral Health Case Manager - Program of Assertive Community Treatment 
 

As a Case Manager / Care Coordinator, you will function as a member of a multidisciplinary team while demonstrating the clinical skill and experience to plan, develop, coordinate, and provide treatment, rehabilitation, and support services to program clients.  To provide education, consultation to families, and crisis intervention services to clients assigned to the team.

WHAT YOU'LL DO

  • Provide care coordination services for an assigned group of clients. Provide direct services such as individual, group, family counseling, crisis intervention, and case management to assigned clients.
  • Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in collaboration with the treatment team.
  • Educate and support clients' families and advocate for clients' rights and preferences.  Consult with families and community agencies such as DSHS, court systems, housing authorities, etc., to maintain coordination in the treatment process.
  • Teach assigned clients skills such as activities of daily living such as personal hygiene and grooming, nutrition, grocery shopping, and food preparation.
  • Assist clients in finding and maintaining a safe, affordable place to live, to include supporting clients in household activities such as house cleaning and laundry.
  • Ensure that clients have adequate financial support (e.g., help to gain employment or to secure entitlement benefits). Assist clients with budgeting and bill-paying skill

WHAT YOU'LL BRING

  • Bachelor's degree in Social Work, Counseling/Psychology, or a related field required.
  • Minimum of one-year experience working with clients with mental illness required.
  • Reliable transportation required
  • Excellent communication skills both verbally and in writing.
