Behavioral Health Case Manager - Program of Assertive Community Treatment
As a Case Manager / Care Coordinator, you will function as a member of a multidisciplinary team while demonstrating the clinical skill and experience to plan, develop, coordinate, and provide treatment, rehabilitation, and support services to program clients. To provide education, consultation to families, and crisis intervention services to clients assigned to the team.
