Navigator (Healthcare)



Heluna Health invites applications for a full-time PrEP/PEP/Doxy-PEP Navigator to work in the San Francisco Disease Prevention and Control Branch, Population Health Division as part of the SF City Clinic (SFCC) PrEP/PEP/Doxy-PEP Team. The SFCC PrEP/PEP/Doxy-PEP program is focused on providing biomedical sexual health prevention services to our clinic patients, including oral and injectable HIV pre-exposure prophylaxis (PrEP), HIV post-exposure prophylaxis (PEP), and doxycycline post-exposure prophylaxis for STI prevention (doxy-PEP). The position offers the opportunity to have a direct, tangible impact on the HIV/STI epidemic in San Francisco and on promoting health equity for our priority populations for sexual health services, by delivering highly effective biomedical prevention options to patients. DPC has a strong focus on health equity.

Under direct supervision of the Biomedical HIV/STI Prevention Coordinator, the PrEP/PEP/Doxy-PEP Navigator will provide sexual health counseling and education to patients, navigate them to options for coverage of the cost of PrEP, PEP, and doxy-PEP, and provide solutions to patients to enable them to remain engaged in sexual health care.

This is a temporary, grant-funded, full-time, benefitted position. Employment is provided by Heluna Health.

This position works in-person at the clinic full time, non-remote

Pay rate: $ 28.09-$35.00 per hour

ESSENTIAL FUNCTIONS

 

 

Clinic-based duties

 

 

  • Perform patient HIV pre-exposure prophylaxis (PrEP), HIV post-exposure prophylaxis (PEP), and doxycycline post-exposure prophylaxis for STI prevention (doxy-PEP) counseling and education to clinic patients
  • Provide consultations and respond to inquiries about PrEP, PEP, and doxy-PEP
  • Refer patients to appropriate medical and social services
  • Provide negative HIV rapid test results to patients
  • Track, stock, and maintain clinic supplies. This involves bringing boxes of supplies up/down 1-3 flights of stairs.
  • Maintain organized and accurate paperwork, both printed and digital
  • Receive, enter, and organize lab results for clinical review
  • Monitor PrEP, PEP, and doxy-PEP phone line used by patients
  • Assist with enrollment into patient assistance programs, respond to prior authorization requests and perform benefits navigation
  • Provide information and referrals on health insurance
  • Follow up with clients in the PrEP, PEP, and doxy-PEP program through phone-based outreach
  • Assist as backup registration worker when additional staffing needed in the registration unit
  • Participate in team and clinic-wide quality improvement (LEAN) initiatives
  • Maintain records incidental to other assigned duties, including patient's charts; may conduct surveys and operates technical equipment.

 

 

Community-based duties

 

 

  • Conduct outreach to members of the priority population with a history of STIs and offer them PrEP and doxy-PEP education and referrals as part of the “data to PrEP/doxy-PEP” program
  • Help organize and speak at PrEP‐related Town Halls (presentations at churches, community centers, bars, etc.) and participation in events such as the Gay Pride parade or Castro Street Fair
  • Provide PrEP, PEP, and doxy-PEP information through other community‐based services
  • Build and maintain relationships with community-based service providers
  • Conduct outreach at encampment health fairs and other non-conventional sites, such as jails and street-based outreach
  • Participate as member of consortium of PrEP, PEP, and doxy-PEP and HIV care navigators, attend regular meetings and share best practices
  • Communicate clearly and in a professional manner with staff throughout the Population Health Division, with the Getting to Zero consortium and with consumers and clinicians

 

 

Other duties:

 

 

  • Maintain a detailed and organized schedule of tasks and events
  • Respond creatively and professionally to challenges that arise in the course of the work
  • Meet with supervisor weekly and as needed to report progress and solve problems
  • Assure ethical practices and maintain confidentiality
  • Ensure that all patients' rights are protected
  • Attend and actively participate in Unit and All Staff Meetings
  • Perform all work activities per established protocols and procedures
  • Wear proper PPE as mandated per COVID-19 mitigation guidelines
  • Other related duties as assigned

 

 

 

JOB QUALIFICATIONS

 

Education/Experience

 

  • One year of experience working in a social services, public health, or health care setting
  • Experience with culturally, economically, and racially diverse populations, including queer and trans people, BIPOC (Black, Indigenous, and other people of color), and people experiencing homelessness
  • Background knowledge of sexual health, STIs, and HIV desired but not required

 

 

Other Skills, Knowledge, and Abilities 

  • Ability to work effectively with coworkers and clients of diverse races, ethnicities, ages, gender identities, and sexual orientations in a multicultural environment
  • Ability to provide a welcoming environment for queer and trans patients
  • Ability to operate a computer and proficiency in the use of Microsoft Office Suite software including Microsoft Word and Excel
  • Comfortable discussing sexual behavior in a clear and professional manner with patients and other staff
  • Attention to detail; strong interpersonal skills; excellent, effective verbal and written communication skills to coordinate with subjects, team members, other departments and outside institutions
  • Ability to multi-task in a fast-paced environment while working with a diverse patient population
  • Ability to collaborate with people of different professional disciplines

Address: San Francisco City Clinic, 356 7th Street, San Francisco, CA. This position works in-person at the clinic full time, non-remote. Involves direct patient service. Will also work at community venues, including street and encampment-based outreach.
