WORKING CONDITIONS: · Sits, stands, bends, stoops and walks intermittently during the workday · Push and pull carts and wheelchairs · Ability to lift and move residents and equipment · Some weekends and holidays required · On occasion, may be required to work in other wings/units other then "home" unit AREA OF RESPONSIBILITY: · Care for residents' needs, as identified through resident assessments, and described in the resident's plan of care · Responsible during the shift for the total nursing care of residents in assigned unit · Provide professional nursing care to residents, following established facility policies and procedures · Assist supervisors in care and management of the unit and providing quality care. ESSENTIAL FUNCTION / DUTIES: · Assist in implementing resident care policies and procedures · Assess, evaluate, plan, and implement resident care plans and respond to resident needs · Ensure communication with family in the event of patient transfer to hospital or post acute facility · Ensure appropriate documentation of medication discrepancies · Chart Compliance Reviews · Ensure monthly documentation and review of risk indicator reports with specific areas of infection control, incidents, skin concerns and medication errors. · Check rooms routinely to insure residents are being provided the care requested · Delegate responsibility as needed to nursing personnel · Maintain charts, make observations · Order supplies and equipment, carry out instructions of supervisors, and observe departmental safety standards and regulations. · Able to recognize significant changes in the conditions of residents and take necessary action. · Supervise direct resident care personnel in the unit. · Ensure that the individual nursing care plan is followed. · Administer or supervise the preparation and administration of prescribed medications. · Administer or supervise prescribed treatments. · Supervise serving of prescribed diets and fluid intake (it is acceptable to document only deviations from normal) and report persistent unresolved problems to the physician and the Director of Nursing. · Ensure that all medications and treatments charted appropriately by the person administering the medication or completing the treatment on that person's assigned shift. · Supervise the preparation of incident and accident reports. · Approve and sign each chart when observations are made. Each chart must be signed in accordance with State standards. · Ensure that a copy of the medical record or an abstract of it is forwarded with the transfer of resident. · Ensure that drugs covered by the Controlled Substances Act are verifiable by inventory. · Assure appropriate destruction of medications. · Participate in regular staff meetings. · Responsible for the detection and correction of situations that have a high probability of causing accidents or injuries to residents. · Provide nursing care relevant to condition and prescribed care as frequently as necessary to maintain quality of resident care. · May perform minor housekeeping duties around the nursing unit. · Perform routine charting. · Assist aides and orderlies as necessary. · Instruct resident in self-help and self-care. · Other duties as assigned
