Intake Case Manager (Personal Services)



Job Description:

This position provides a comprehensive and holistic intake assessment to all incoming program participants to Human Services. The Intake Case Manager will identify and document the circumstances and needs of incoming participants, matching them with the appropriate internal housing navigator, and complete community referrals as needed. The position reports directly to the Director of Human Services Central. This position requires effective interaction with participants, community resources, staff members, volunteers, and other agency representatives.  The position also involves coordination with relevant community services to support program participants' success. 

Essential Functions:

Examples of work include, but are not limited to the following:

  • Conduct screenings and intakes.
  • Engage in regular case management appointments.
  • Analyze needs and resources.
  • Complete needs assessments.
  • Maintain a limited caseload of persons experiencing or at risk of experiencing homelessness.
  • Provide case management services including advocacy, referrals, and retention.
  • Monthly housing retention appointments with participants.
  • Develop relationships with landlords in the community.
  • Liaison with external referral sources and community partners.
  • Maintain accurate and timely case records, reports, and statistical data.
  • Interact with low-income populations.
  • Maintain confidentiality within and outside of the agency.
  • Work independently using initiative, creativity and flexibility in meeting participant needs while preserving their dignity.
  • All other duties as assigned.

Education and Experience:

  • 2-4 years' experience working with individuals/families experiencing or at risk of homelessness.
  • Valid driver's license and access to a vehicle on a continual basis.
  • Experience providing holistic case management services.
  • Experience interacting with volunteers.
  • Knowledge of Washington County Social Services networks.
