Nurse Navigator - Pediatric Neurology Clinic (Healthcare)



OU Physicians is Oklahoma's largest physician group and encompasses almost every adult and child specialty.

 

POSITION INFORMATION:  Under general administrative direction, coordinate referrals from physicians and health care facilities for patients; provides a significant level of patient education related to their illness and planned treatment.  

 

REPRESENTATIVE DUTIES: 

  1. Patient Coordination.  Works closely with physicians in all specialty areas to coordinate patient's care plan communication; works with multidisciplinary team to maintain and implement up-to-date coordinated care plan; communicates with all members of the healthcare team on behalf of the patient
  2. Patient Referral.  Uses clinical protocols to refer patients to appropriate specialist for diagnosis or treatment and assists patients and their families in obtaining referrals to needed specialists.  Provides support and counseling, as appropriate to the clinical situation.
  3. Clinical Protocol Development.  Works with designated physicians to develop and maintain clinical protocols.
  4. Clinic Improvement.  Proactively develops and enhances processes with the goal of improving the clinical experience for referred patients and the referring physician.
  5. Liaison.  Communicates with referring physician's offices as appropriate to the patient and physician's needs.
  6. Patient Education.  Works with marketing and outreach departments to educate referring physicians and facilities on available services.
  7. As Needed.  Performs various duties as needed to successfully fulfill the function of the position.

Education:  Bachelor's Degree in Nursing

 

Experience:  24 months experience in nursing (RN) as a health educator

 

Equivalent/Substitution: Will accept an Associate's Degree in Nursing and 24 months RN-level nursing experience in lieu of the Bachelor's Degree in Nursing for a total of 48 months

 

Certifications/Licenses:  Registered Nurse Licensure in the State of Oklahoma

 

Skills: Strong communication skills (both oral and written)

 

Preferred: Master's Degree in Nursing or Public Health (MSN/MPH)
