RN Patient Care Coordinator (Full-time, 10a-6p M-F/ Varied Days, Possibility of every 3rd Saturday) (Project Management)
SUMMARY The Patient Care Coordinator provides ongoing support and expertise through comprehensive assessment, planning, assisting providers with order entry, implementation, teaching, and overall evaluation of individual patient needs. The overall goal of the position is to support the ideal of patient-centered care and to promote coordination of care and cost effectiveness through integrating the functions of case management, nursing process, utilization management, discharge planning, and discharge teaching. Has accountability for coordination of care and discharge planning of all patients in conjunction with the entire care team. Compliance with rules and regulations of all applicable federal, state, and local laws as well as PMHC policies is a condition of employment.
ESSENTIAL FUNCTIONS Incumbent must have the skills, ability and judgment to perform the following essential job duties and responsibilities with or without reasonable accommodation: 1. 1. Serves as a patient advocate. Enhances a collaborative relationship to maximize the patient's and family's ability to make informed decisions. 2. 2. Coordinates the integration of social services/case management functions into the patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies and healthcare facilities. 3 3. Conducts concurrent medical record review using specific indicators and criteria (Interqual) as approved by medical staff, CMS, and other state agencies. 4. 4. Mobilizes and promotes the effective and efficient utilization of clinical resources. 5. 5. Supports quality patient care, e.g. core measures and other evidence based practice guidelines by supporting and communicating with entire care team. 6. 6. Develops an effective working relationship and communicate with physicians at regular intervals throughout hospitalization to coordinate care of patients. 7. 7. Assists physicians to maintain appropriate cost, case, and desired patient outcomes. 8. 8. Assists providers with documentation and order entry needs including ensuring and oversight of correct entry into the medical record. 9. 9. Initiates and presents "denial letters", as appropriate. 10. Establishes a relationship with patients and their families and complete an expanded assessment of their needs as soon as possible after admission (ideally day 1). 11. Facilitates interdisciplinary patient care rounds and/or conferences to review treatment goals, optimize resource utilization, provide family education and identify post-hospital needs. 12. Collaborates with clinical staff in the development and execution of the plan of care and achievement of patient-centered goals. 13. Develops, implements, and teaches the discharge plan. 1 14. Coordinates the provision of social services to patients, families, and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from healthcare services. 15. Adheres to departmental goals, objectives, standards of performance, and policies and procedures. 16. Maintains patient and employee confidentiality at all times. 17. Complies with PMHC Code of Conduct.

REQUIRED EDUCATION/EXPERIENCE 1. Bachelors, Associate, or Diploma in Nursing from an accredited college or university. 2. Minimum of two years of hospital clinical experience. 3. Minimum of two years utilization review/case management experience or social work experience preferred.
COMPETENCIES/CERTIFICATIONS REQUIRED 1. Current RN from the state of Nebraska is required. Nursing license must be active for the state of Nebraska. 2. Certification in Case Management (e.g. CCMC) is preferred. 3. Current BLS certification.
