Field RN (Finance)
Demonstrate proficient skills to complete home visits for a group of chronically ill patients. The RN's primary responsibility is to complete in-home initial assessments and risk stratification of patients referred to Alegis. The RN will initiate and contribute to care plans and care goals for each patient.
Secondary responsibilities will include post discharge home visits, where the RN will ask pertinent discharge triage questions and complete a post DC home safety evaluation that includes medication reconciliation. The RN will document all assessment findings. The RN will escalate issues that threaten patients' well-being in the home. The
RN will assist in the coordination of care and services as needed and directed by the Care Team. This may involve follow up calls, and collection of relevant patient information. The RN will also perform interim home visits as directed by the Care Team. The RN may also pass out home stool kits, and administer vaccines as needed. Field nurse will support all lines of Alegis Care business including our chronic care management programs, transition of care program, and palliative care program.
Qualifications/Educational Requirements :
Minimum of 5 years of nursing experience as an R.N.
Responsibilities/essential functions : The person in this position must be able to perform the following essential job functions with or without reasonable accommodations. This position includes travel via car in a 60 mile radius in the Nashville area, walking in and out of pts homes, performing patient home assessments.
1. Perform in home visits
3. Observe, assess, and document.
4. Monitor patient progress.
5. Educate patients and caregivers on disease process, medications, plan of care, and individualized care goals.
6. Coordinate patient services in conjunction with our office based RNs, Care Associates, NPs, physicians, and other care team members.
7. Notify the physician and other personnel of change in the patient's condition.
9. Participate in patient care rounds and interdisciplinary care team meetings to facilitate coordination of care.
10. Update clinical records according to policy and procedures.
11. RN may be asked to do telephonic care coordination for programs if there is a need. This can include: outreach and enrollment, triage and orders management.
