Clinical Case Manager LPHA CORC (Personal Services)



About Us:

Volunteers of America Southwest is looking for a dynamic person to help create change in our communities through our Comprehensive Opioid Recovery Center (CORC). The CORC program is a unique program designed to assist individuals in dealing with opioid addiction. The CORC program is a multifaceted program that provides services that match our client's level of need. The program is fluid and dynamic in providing services that include inpatient and outpatient treatment, case management, supportive services, benefit enrollment assistance and supportive housing services. Our Clinical Case Manager (LPHA) will help us in developing our program and to provide quality services to the community.

Job Opportunity:

We are looking for a Clinical Case Manager - LPHA to join our Opioid Recovery Program. The Volunteers of America mission is to help individuals reclaim their lives from the disease of addiction and to reduce the incidence of addiction in future generations. We are looking for applicants who have experience working in treatment and with the homeless population. We are an agency committed to quality, responsive, trauma-informed care to adults with substance use and co-occurring disorders. The Clinical Case Manager - LPHA provides critical drug assessment and referral services to clients referred to the Volunteers of America Comprehensive Opioid Recovery Center program.

Key Responsibilities:

  • Utilizing information gathered at intake and through ongoing assessment, collaborate with the client to determine needs and create individualized case management service plans for clients.
  • Responsible for completing initial and updated case management plan within the specified time frames for all clients on your caseload.
  • Facilitating therapeutic and self-help groups in accordance with the group calendar.
  • Maintain regular progress notes, records, data entry, and other documentation as required.
  •  Comprehensive assessment and periodic reassessment of individual needs to determine the need for the continuation of services; revision of client case management plan when necessary.
  • Actively help clients to apply for and maintain health and public benefits (i.e., Medi-Cal, Healthy San Diego, Perinatal, Housing, etc.).
  • Link clients to community resources and services (i.e., transportation, food and clothing assistance, family planning services, legal assistance, educational resources and job training, exc.).
  • Provide direct services to clients, including one-on-one meetings, to facilitate referrals in support of clients' treatment plan goals (i.e., psycho-social-medical services; HIV and TB testing and counseling; assistance obtaining driver's license, birth certificates, and Social Security cards; medication-assisted treatment; supportive housing, insurance).
  • Provide recovery support and care coordination to enhance treatment completion including engagement, outreach, clean and sober socialization, life skills workshops, and other groups as required.
  • Coordinate care with physical health, community health clinics and providers, and mental health providers,
  • Engage clients in his/her treatment and recovery by identifying and developing additional supports such as linkages to self-help groups, peer support activities, and recovery monitoring and coaching.
  • Advocate for clients across various systems to ensure appropriate and timely linkages to physical and mental health care, transportation, and retention in primary care services.
  • Identify resources, conduct outreach, develop relationships with community partners, and maintain up-to-date resource directories to ensure clients have access to needed services.
  • Facilitate warm hand-offs where appropriate.
  • Attend all required meetings and trainings and maintain up-to-date certifications to ensure service delivery is ethical, culturally competent, gender-responsive, trauma-informed, and evidence-based.
  • Adhere to all personnel requirements such as the agency code of conduct, and policies detailed in the employee handbook including, but not limited to dress code, timely reporting, and certification/training requirements.

Qualifications, Education and Experience:

Must be a licensed Marriage and Family Therapist or a Licensed Clinical Social Worker in the State of California. Past experience should demonstrate at least two years of behavioral health experience; proven ability to work across multiple systems such as probation, criminal justice, associate clinical social worker, marriage and family therapist, professional clinical counselor etc.; and strong knowledge of and relationships with community resources. Skills include excellent written and verbal communication, ability to manage multiple tasks in a fast-paced environment, strong documentation, and proven success as a team player. Must comply with all agency and contract requirements related to trainings, public health, certifications, reporting, etc. Must be able to pass a drug test, background clearance, and provide a negative TB test. Individuals with bilingual proficiency preferred.
