Under the direction of the RN, assist in identifying the patient's physical, psychosocial and environmental needs as evidenced by documentation, clinical record, team report and evaluations. 2. Participate in planning and implementing care in conjunction with the RN, in accordance with the POC. 3. Provide care utilizing infection control measures that protect both staff and patient (OSHA) 4. Assure the continuity of care through delivery of quality patient care. 5. Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conference, communication notes and evaluations. 6. Monitor assigned cases to ensure compliance with requirements of third part payor. 7. Demonstrate commitment, professional growth and competency. 8. Promote agency philosophy and administrative policies to ensure quality care. 9. Participates in Utilization Review, Quality Improvement activities and chart audits/reviews. 10. Adheres to personnel policies/procedures, i.e. dress code, correct mileage reporting and documentation, professional work ethics. 11. Attends and/or participates in mandatory department requirements: Annual Fire Drill, Health screen & TB Testing, License Renewal, Blood and Body Fluids, Emergency Preparedness, General Safety, Airborne Transmission, Fire Safety, Electrical Safety, Fall Prevention and Infection Control. 12. Is a team player, strives to work harmoniously with other members of the team, shows interest in learning new skills and procedures, and demonstrates a genuine interest in Home Health, department productivity and its goals. Makes his/her supervisor aware of problems/issues that may lead to poor workmanship and is willing to ask for assistance from the Employee Assistance Services (EAP) if and when necessary. 13. Is aware of customer satisfaction and the procedure and the process to follow for a complaint. Is aware of agency's intention of providing high quality patient care which is demonstrated by the following: a. Is courteous to patients, visitors and employees in the agency b. Treats patients/family members and their property with respect c. Make sur patients are aware of their rights and responsibilities d. Handles inquires and complaints in an appropriate manner e. Has had no valid complaints from patients, family members and visitors in the past year. f. Creates a Positive First Impression
GENERAL FUNCTIONS:
1. Maintain confidentiality of client, employee and facility information. 2. Follow Client's Rights policies at all times maintaining privacy, confidentiality and respect. 3. Ability to understand and comply with applicable Federal, State and local regulations as they apply to this job. 4. Attend all required department and/or facility meetings. 5. Adhere to applicable laws, rules, regulations and Code of Conduct directed by Corporate Compliance Program, conduct business using ethical standards, and report any suspect or actual event or circumstance to the Compliance Officer. 6. Understand and comply with use and disclosure rules of resident Protected Health Information (PHI) as required by the HIPAA Privacy Rule of the Health Insurance Portability and Accountability Act. 7. Use the proper equipment for each job and report any missing or damaged equipment to supervisor immediately. 8. Follow facility safety program, identify and report unsafe equipment, environment or procedures to supervisor. 9. Assume accountability for data contained in the employee handbook. 10. Observe all agency policies and procedures. 11. Report to work as scheduled and consistently demonstrate dependability and punctuality. 12. Come to work in neat, clean attire and consistently present an appropriate professional appearance. 13. Perform all duties in an effective, timely and professional manner. 14. Work cooperatively with all community members* and willingly accept assigned duties. 15. Report any possible problem or concern regarding community members* to supervisor immediately. 16. Communicate applicable information to community members* in an effective and efficient manner. 17. Exhibit hospitality by displaying courtesy, support, gratitude and respect to all community members*. 18. Complete employee training courses as assigned by due date. 19. Participate in working nurse rotation. 20. Perform any other related duties as directed by the Clinical Director.
