Responsibilities:

• Provide direct nursing care to residents, following care plans and physician instructions
• Monitor and document patient conditions, vital signs, and medical histories
• Administer medications and treatments as prescribed
• Collaborate with the healthcare team to ensure a safe and supportive environment
• Communicate effectively with patients, families, and staff

Qualifications:

• Current CA LVN license in good standing
• Previous experience in post-acute or long-term care preferred
• Strong clinical skills with the ability to work well under pressure
• Compassionate, patient-centered approach to care
