Summary: The Personal Care Charge Nurse provides prescribed medical treatment and services to older adults in the Personal Care Home setting. The Personal Care Charge Nurse also coordinates physical, social, and emotional aspects of care and services in accordance with the physician's orders and the Resident's individualized Support Plan. The Personal Care Charge Nurse contributes to building a positive team spirit and supports everyone's efforts to succeed. Homeland Center has over 150 years of committed service to the aging population.
Essential Duties & Responsibilities:
Ensures appropriate documentation on all clinical records, including computerized medication system.
Ability to speak professionally with physicians and able to transcribe physician orders completely and accurately.
Observing and assessing residents daily for changes in condition and needs.
Participates in the development of the individualized support plan for residents.
Completes medication passes and treatment schedules to ensure that medications are being administered, as ordered, and that treatments are provided, as scheduled.
Makes emergency and critical care decisions involving residents and staff; keeping Clinical Manager/Director of Personal Care informed.
Ensures that CNA/Caregivers provide assistance with activities of daily living and assists when necessary.
Assisting with Admission, Transfer, and Discharge assessments and paperwork as needed.
Performs all nursing duties as required by adhering to educational training, licensure standards, facility policy and procedure, and Department of Human Services regulations
Other duties which may be assigned.
Expectations:
· Must be able to effectively and efficiently administer medications to the Residents and coordinate their medical needs.
· Must able to accurately observe, document and report any significant changes in a Resident's condition.
· Must proactively provide supervision, insight and sound guidance to Certified Nursing Assistants and Medication Assistants.
· Must be able to effectively communicate wellness and medical regulations, policies and procedures to Residents, families, vendors, and team members.
· Must be firm, direct, and self-assured in dealing with minor, as well as highly sensitive issues and emergencies.
· Must be able to communicate with a sense of customer service and professionalism, while maintaining the ability show empathy and compassion.
· Must be able to display willingness to make decisions; Exhibits sound and accurate judgment.
· Must be willing to improve job knowledge by maintaining licensure and attending training sessions.
