TRANSITIONAL CARE SPECIALIST (Finance)
Act as a liaison between the agency and healthcare providers, including interaction with case management and discharge planners at hospitals, skilled nursing/rehabilitation facilities, physicians and other healthcare providers to provide education about the agency's home health health/hospice/PAS services, the benefits of care and clinical outcomes.
Develops short-term and long-term sales strategies to establish and/or expand business in the assigned region.
Implements process to determine which referrals are directly related to the contact of the specialist.
Develops marketing plans, business plans, sales strategies and action plans for identified targets of opportunity that clearly define objectives, goals, win strategies, schedules and action assignments.
Develop and expands existing client base.
Participates in all health fairs for the region.
Promotes business by continuously calling and identifying potential clients.
Establish and maintains on-going client relationships with discharge planners, physician staff and physicians; hospitals, nursing homes, residential centers, senior citizen centers and other sources as requested by the Manager or Executive Director.
Provide detailed feedback regarding sales calls or prospects to the manager and/or Executive Director.
Oversees public relations activities, special events and celebrations for Home & Community Services.
Monitors, evaluates, promotes and orders the promotional products offered by the agency.
Conducts presentations to appropriate associations and organizations to increase awareness of the agency.
Meets quota for episodic business as indicated by personal goal plan, based on salary and territory size, set by BDM.
Assist other Specialist as needed to provide direction and communication.
Conduct pre-discharge hospital patient visits at the healthcare provider's request to determine the need and eligibility for home health/hospice care, introduction to the agency upon acceptance of a healthcare provider's referral and explain available home health, hospice and PAS services to patients and families.
Assist agency staff in the timely procuring and processing of physician's orders for home health/hospice care assessment, including obtaining a patient's current history and physical information and any other pertinent information to assist in the acceptance of the referral
Coordinates home health/hospice/PAS admissions to the agency on an as needed basis.
Performs facility courtesy visits to coordinate discharging and admission as requested by referral sources.
Performs community education activity to hospitals, physician offices, etc. on an as needed basis.
Coordinates equipment set up for new admissions as needed.
Performs Client referral follow ups for all admissions.
Coordinates and assists the interdisciplinary team in the development and implementation of patient care policies.
Tracks/coordinates and follows all patient facility admits.
Completes Intake progress notes as appropriate.
Assists in developing educational activities for staff including in-services based on employee needs and promotes staff development.
Any other duties as assigned by the Executive Director/Administrator or the Senior Management Team.
