Case Manager III - Various Locations (Finance)
Identify Housing based on client needs
Facilitate rent and move in assistance
Provide support services to support housing
Works closely with Housing Resource Team to identify housing and to engage rental market
Engages initially with clients who are experiencing homelessness and currently residing in a partner shelter.
Carry a caseload made up of clients currently experiencing homelessness and clients who are housed.
Provides services focused on assisting clients to maintain permanent housing.
Facilitate USHS applications as needed.
Work collaboratively with
Works closely with Property Management to address any issues related to lease violations.
Monitor and meet funder outcome requirements.
Participates and leads in case conferences as needed.
Participates and leads in Community Housing trainings and activities to foster team relationships.
Assist case managers with difficult clients.
Provide assistance and review of DCAs.
Will act in the place of the Program Director as necessary.
Provide guidance and support to case managers.
Provides support to the Program Director regarding reporting, program administration and quality assurance.
Conducts ongoing audits of files to ensure that files are current and accurate.
Develop individualized housing stability plans, including post housing stabilization plan.
Coordinate client services through referrals with other social services, mental health, medical, employment, educational, child care and other providers based on client needs and goals.
Maintains a case load utilizing housing first, trauma informed and client centered approaches, while building effective, collaborative relationships with clients, monitoring and documenting client progress toward service plan goals; ensures accuracy of files for audits and program outcomes.
Documents all client interactions following CARF required documentation.
Conduct regular case conferences with clients and service providers as required.
Develop familiarity with unique challenges and resources as it relates to homelessness.
Secures release of information from clients when needed/requested in order to mediate conversations between clients and their family and/or other service providers to assist in resolving a client's housing crisis, and to coordinate wrap around services for clients.
Ensures goals are being met.
Other duties as assigned.
