Social Worker - Hospice (Administrative)
The Hospice Social Worker identifies social and emotional problems impacting the patient's medical condition and helps the patient to establish goals along with developing a social work plan of care. Knowledge and use of community resources is essential in addition to close collaboration with members of the hospice team and coordination of care with community agencies and other health care providers. Hospice social work documentation of evaluation visits and progress notes are completed as part of the patient's Medical Record. The hospice social worker participates in discharge planning, interdisciplinary conferences, in-service programs and acts as a resource for other hospice team members.
